The report, made public Tuesday, describes chaos and poor coordination as a gunman roamed Terminal 3. Emergency alert systems like panic buttons didn't work and the radios emergency responders carried weren't compatible between agencies. It took a cell phone call to give dispatchers a critical piece of information: where the shooting was happening.
