Federal officials say the sickest five percent of Americans rack up more than half of all healthcare costs. These people often have chronic health conditions, and have high rates of emergency room visits and hospitalizations. A program in San Diego is attempting to teach these so-called "high utilizers" how to better navigate the healthcare system, and manage their own conditions.
