When doctors treat their most vulnerable patients, they know plenty of things are beyond their control. Patients may misunderstand medication instructions, or have trouble getting to their appointments. Los Angeles County is experimenting with a low-tech solution to help. Community health workers spend time with patients and help them better manage their own health.
